Terms of Service Agreement

1. Nature of Services and Scope of Practice

Integrative Health Solutions (IHS), LLC provides wellness education, functional health coaching, lifestyle support, stress management strategies, nutritional guidance, and general health-building recommendations. These services are not medical care. We do not diagnose, treat, or manage medical conditions. All information is educational and informational only. You agree to consult a licensed medical provider for medical concerns, diagnoses, medication adjustments, or treatments.

2. Lab Testing and Interpretation

We may facilitate access to laboratory testing through third-party laboratories. These tests are for wellness purposes only and are not intended for medical diagnosis or treatment. Interpretations are

holistic, functional, and educational. Only your licensed medical provider may interpret labs for medical purposes or provide treatment.

3. Client Responsibilities

You agree to consult your medical provider before making changes to medications, supplements, exercise, or diet. You agree to seek immediate medical care for concerning symptoms, provide

accurate information, and understand that results vary based on individual participation.

4. Payments, Refunds, and Cancellations

All payments are final. No refunds are provided for completed or partially completed services. Payment plans must be completed as agreed. Failure to complete payments constitutes a breach of this

Agreement. Missed or late-cancelled sessions may be forfeited. Chargebacks or disputes are a breach of this Agreement.

5. Intellectual Property

All materials provided by Integrative Health Solutions (IHS), LLC are the intellectual property of the Company. You may not copy, share, distribute, or reproduce materials without written permission.

6. Confidentiality (Non-HIPAA)

Integrative Health Solutions (IHS), LLC is not a HIPAA-covered entity. While reasonable privacy practices are maintained, information shared is not protected under HIPAA laws.

7. Assumption of Risk

You acknowledge that wellness activities involve inherent risks. You voluntarily assume all risks associated with recommendations. Integrative Health Solutions (IHS), LLC is not responsible for adverse reactions, supplement effects, emotional responses, or delays in seeking medical care.

8. Limitation of Liability

To the fullest extent permitted by law, Integrative Health Solutions (IHS), LLC disclaims liability for direct, indirect, incidental, consequential, or punitive damages. Total liability shall not exceed the amount paid for services.

9. No Medical Advice or Diagnosis

All information is educational and does not constitute medical advice. You agree to consult a licensed provider for medical interpretation of labs, diagnosis, treatment, medication adjustments, and clearance for wellness changes.

10. Technology, Communication, and Messaging

We may communicate with you via email, SMS, client portals, or messaging platforms. You are responsible for accurate and updated contact information. SMS messaging requires explicit opt-in.

11. SMS CONDITIONS

1. By opting into SMS messages from Integrative Health Solutions (IHS), LLC, you agree to receive text messages related to appointment reminders, program updates, wellness tips, educational content, client support, promotional offers, and service announcements.

2. You may cancel the SMS service at any time by texting "STOP" to the number you receive messages from. After sending "STOP," you will receive a confirmation message and will no longer

receive SMS messages from us. To rejoin, opt in again through any signup method.

3. If you experience issues with the messaging program, reply "HELP" for assistance or email [email protected].

4. Carriers are not liable for delayed or undelivered messages.

5. Message and data rates may apply. Message frequency varies, generally up to 4–10 messages per month depending on your participation. Contact your wireless provider for details about your text or data plan.

12. Termination

The Company may terminate services at any time for non-payment, inappropriate behavior, or violation of this Agreement. No refunds will be issued for terminated services.

13. Governing Law

This Agreement shall be governed by the laws of the state in which the Company is formed.

14. Entire Agreement

This Agreement represents the full understanding between the parties and supersedes all prior communications.
